Pas de prise de contraste (pas de clich\u00e9 ici = \u00e0 compl\u00e9ter) Compte tenu de l\u2019\u00e2ge, \u00e9vocateur d\u2019un gliome bas grade. }});<\/script>[vc_column_text]<strong>Histoire de la maladie<\/strong><br \/>\nPatiente de 31 ans consulte pour baisse d\u2019acuit\u00e9 visuelle de l\u2019\u0153il droit avec \u00e9clipses visuelles br\u00e8ves et photopsies dans un contexte de c\u00e9phal\u00e9es persistantes depuis 3 semaines. Rapporte \u00e9galement diplopie binoculaire. L\u2019interrogatoire ne retrouve pas de naus\u00e9e ni vomissement. Elle est ambidextre \u00e0 pr\u00e9dominance droite, m\u00e8re de trois enfants, elle pr\u00e9senterait des troubles de l\u2019humeur et du comportement depuis plusieurs mois. Un bilan orthophonique et neuropsychologique confirme les constatations du mari avec des troubles de l\u2019attention et de la m\u00e9moire de travail.<\/p>\n<p><strong>Bilan paraclinique<\/strong><br \/>\nEEG : pas de crise d\u2019\u00e9pilepsie<br \/>\nIRM : l\u00e9sion basifrontale gauche d\u00e9velopp\u00e9e aux d\u00e9pens du gyrus rectus qui remonte vers l\u2019hypothalamus comprimant les foramens de Monro induisant une probable hydroc\u00e9phalie obstructive. Pas de prise de contraste mais s\u00e9quences en perfusion non exploitables.<\/p>\n<p><strong>ATCD<\/strong> : G3P3, p\u00e9ritonites appendiculaires (x3), ob\u00e9sit\u00e9 (1m73 pour 95Kg)<br \/>\n<strong>TTT<\/strong> : aucun<\/p>\n<p><strong>Ex ophtalmo<\/strong><br \/>\nAV = 10\/10 ODG<br \/>\nLAF : segment ant\u00e9rieur calme profond avec cristallin clair.<br \/>\nPIO = 15 mmHg<br \/>\nFO : cf. images jointes<br \/>\nPar\u00e9sie du VI droit (non localisatrice)<\/p>\n<p><strong>Ex g\u00e9n\u00e9ral<\/strong> (non disponible)<br \/>\n<strong>Ex compl\u00e9mentaires<\/strong> (non disponible)<\/p>\n<p>Traitement par SOLUPRED et DIAMOX permettant de contr\u00f4ler l\u2019HTIC. Chirurgie d\u2019ex\u00e9r\u00e8se par la suite. L\u2019anatomopathologie est en faveur d\u2019un astrocytome infiltrant IDH mut\u00e9, grade II OMS, perte d\u2019expression de MGMT, cMET n\u00e9gatif, EGFR fortement positif (= gliome bas grade).
