Diagnostic de troubles hydro-\u00e9lectrolytique (hyponatr\u00e9mie hypo-osmolaire), an\u00e9mie normocytaire inflammatoire avec Sd inflammatoire sans point d\u2019appel clinique de r\u00e9gression spontan\u00e9. D\u00e9nutrition s\u00e9v\u00e8re avec prise en charge nutritionnelle et r\u00e9\u00e9ducation par kin\u00e9sith\u00e9rapie \u00e0 la marche. Au d\u00e9cours du retour \u00e0 domicile, AVC isch\u00e9mique (pas plus d\u2019informations, imagerie c\u00e9r\u00e9brale hors CHU). Consultation qui se fait \u00e0 distance de cet AVC. Confirmation au d\u00e9cours de la p\u00e9rim\u00e9trie cin\u00e9tique des constatations cliniques d\u2019h\u00e9mianopsie lat\u00e9rale homonyme gauche.<\/p>\n<p><strong>ATCD<\/strong> : hypothyro\u00efdie, psoriasis, dyslipid\u00e9mie, AOMI, fractures multiples (cotyle gauche, pertrochant\u00e9rienne gauche, f\u00e9morale droite), cataracte op\u00e9r\u00e9e.<br \/>\n<strong>TTT<\/strong> : kardegic, sotalol, levothyrox, spiriva<\/p>\n<p><strong>Ex ophtalmo<\/strong><br \/>\nAV : 2\/10 Parinaud 8 \u00e0 droite , \u00ab compte les doigts \u00bb \u00e0 gauche<br \/>\nLAF : corn\u00e9e claire, chambre ant\u00e9rieure calme profonde avec implants de chambre post\u00e9rieure en place<br \/>\nPIO : 15 mmHg<br \/>\nFO : DMLA avec drusen et atrophie (pas d\u2019activit\u00e9 n\u00e9ovasculaire)<\/p>\n<p><strong>Ex g\u00e9n\u00e9ral<\/strong> (non disponible)<br \/>\n<strong>Ex compl\u00e9mentaires<\/strong> (non optique (r\u00e9tro-chiasmatique).<br \/>\nPeut faire l\u2019objet d\u2019un dossier pluridisciplinaire g\u00e9riatrie, neurologie et ophtalmologie avec prise en charge du handicap et mise en place
