})();<\/script>[vc_column_text]<strong>Histoire de la maladie<br \/>\n<\/strong>Patiente de 71 ans diab\u00e9tique de type 2 adress\u00e9e dans le cadre du suivi annuel de son diab\u00e8te. Diab\u00e8te de type 2 diagnostiqu\u00e9 en 2003 \u00e0 l\u2019\u00e2ge de 56 ans sur un bilan pr\u00e9 greffe h\u00e9patique. Traitement initial par antidiab\u00e9tiques oraux puis insulin\u00e9 en 2005 avec un sch\u00e9ma basal bolus. Autosurveillance glyc\u00e9mique 3 x\/jour et hyperglyc\u00e9mie le matin \u00e0 jeun.<br \/>\nHypoglyc\u00e9mies rarissimes. Equilibre glyc\u00e9mique difficile en lien avec un non-respect des r\u00e8gles hygi\u00e9no-di\u00e9t\u00e9tiques.<\/p>\n<p><strong>Bilan des complications<\/strong><\/p>\n<ul>\n<li>Ophtalmologique : r\u00e9tinopathie diab\u00e9tique trait\u00e9 par laser, derni\u00e8re consultation en octobre 2018.<\/li>\n<li>Cardiovasculaire : Doppler des membres inf\u00e9rieurs : surcharge intimale calcifi\u00e9e, et Doppler des troncs supra aortiques en 2017 : normale. Pas de suivi cardiologique.<\/li>\n<li>N\u00e9phropathie diab\u00e9tique avec un DFG \u00e0 52, et une microalbuminurie positive \u00e0 15.8.<\/li>\n<li>Pas de neuropathie p\u00e9riph\u00e9rique.<\/li>\n<li>Pieds : pas d\u2019ant\u00e9c\u00e9dent de plaie.<\/li>\n<\/ul>\n<p><strong>ATCD<\/strong>\u00a0:<\/p>\n<ul>\n<li>Ant\u00e9c\u00e9dents m\u00e9dicaux : diab\u00e8te de type 2, HTA, dyslipid\u00e9mie, AOMI, insuffisance r\u00e9nale chronique, abc\u00e8s testiculaire gauche<\/li>\n<li>Ant\u00e9c\u00e9dents chirurgicaux : hernie ombilicale, hernie inguinale \u00e0 gauche, greffe h\u00e9patique sur cirrhose en 2003, orchidectomie gauche en 2017, cataracte bilat\u00e9rale op\u00e9r\u00e9e<\/li>\n<li>Ant\u00e9c\u00e9dents familiaux : non connu<\/li>\n<li>Pas d\u2019allergie connue.<\/li>\n<li>Pas de tabac, alcool sevr\u00e9 depuis la greffe en 2003.<\/li>\n<\/ul>\n<p><strong>TTT<\/strong><\/p>\n<p>NOVORAPID 16-14-14U<br \/>\nABASAGALR 28U le soir<br \/>\nKARDEGIC 75 1 cp le matin<br \/>\nAPROVEL 300 1 le matin<br \/>\nCRESTOR 10 1 le soir<br \/>\nMYFORTIC 360 2 cp le matin et le soir<br \/>\nUVEDOSE 100.000<u><\/u><\/p>\n<p><strong>Ex ophtalmo<\/strong><\/p>\n<p>AV : 9\/10<br \/>\nLAF : corn\u00e9e claire, chambre ant\u00e9rieure calme profonde, pas de rub\u00e9ose irienne, cataracte d\u00e9butante bilat\u00e9rale<br \/>\nPIO : 15 mmHg<br \/>\nFO : cf. supra<\/p>\n<p><strong>Ex g\u00e9n\u00e9ral<\/strong><\/p>\n<p>Poids : 78 kg ; Taille : 169 cm ; IMC : 27.3 kg\/m\u00b2 ; Tension art\u00e9rielle : 135\/77 mmHg ; Fr\u00e9quence cardiaque : 82 bpm. Temp\u00e9rature : 36.7 \u00b0C. SAT : 95% en air ambiant. Glyc\u00e9mie \u00e0 l\u2019entr\u00e9e : 4 g\/l.<br \/>\nBruits du c\u0153ur assourdis. Pas de souffle carotidien. Pouls distaux au niveau des membres inf\u00e9rieurs sont faiblement per\u00e7us. Mollets souples et indolores. Pas de signe d\u2019insuffisance cardiaque droite. Claudication \u00e0 la marche. Auscultation pulmonaire sans particularit\u00e9. Abdomen souple, d\u00e9pressible et indolore. Absence d\u2019angiome stellaire, absence d\u2019ict\u00e8re, absence d\u2019hippocratisme digital. L\u00e9g\u00e8re \u00e9rythrose palmaire et plantaire. Mauvais \u00e9tat bucco-dentaire.<br \/>\nSur le plan neurologique : patient conscient, coh\u00e9rent, orient\u00e9.
