Il \u00e9tait debout sur un tabouret pour changer une ampoule \u00e9lectrique dans la cuisine quand il a perdu l&rsquo;\u00e9quilibre, est tomb\u00e9 sur le sol carrel\u00e9 et s&rsquo;est frapp\u00e9 la t\u00eate. Il a perdu connaissance pendant moins d&rsquo;une minute. Il a remarqu\u00e9 une diplopie d\u00e8s son r\u00e9veil, mais ne pr\u00e9sentait aucun autre sympt\u00f4me neurologique. Il s&rsquo;est pr\u00e9sent\u00e9 \u00e0 son service d&rsquo;urgence local pour \u00e9valuation. L\u2019imagerie par r\u00e9sonance magn\u00e9tique (IRM) de son cerveau n\u2019\u00e9tait pas r\u00e9v\u00e9latrice et la tomodensitom\u00e9trie (TDM) ne montrait aucune fracture orbitaire.\u00a0Sa diplopie a persist\u00e9, ce qui a motiv\u00e9 sa premi\u00e8re visite. La diplopie ne s&rsquo;est pas d\u00e9t\u00e9rior\u00e9e ni am\u00e9lior\u00e9e depuis son apparition. Il d\u00e9crit une diplopie verticale indolore, binoculaire et verticale qui s\u2019aggrave lorsqu&rsquo;il regarde \u00e0 gauche, mais qui s&rsquo;am\u00e9liore lorsqu&rsquo;il penche la t\u00eate vers la gauche. Il pense que la diplopie est pire quand il essaie de lire.
